Testicular Sperm Extraction

Testicular sperm extraction (TESE) is a procedure that is performed under general anaesthesia, or occasionally light sedation. This procedure is often performed in individuals who did not have any sperm identified during a PESA procedure, or in individuals who do not have sperm in their ejaculate.

Testicular Sperm Extraction

TESE, which stands for Testicular Sperm Extraction, is a medical procedure utilised to gather sperm cells directly from the testes. This process is necessary when there are no sperm cells present in the semen, the fluid in which sperm cells reside. TESE may be required if:

  • Your testes are not producing enough sperm cells in your semen
  • A blockage prevents your sperm cells from exiting your penis during ejaculation, which occurs when semen is released from your penis after orgasm
  • You have certain medical conditions that affect the production or transportation of sperm cells

For men with these conditions, in vitro fertilization (IVF) with intracytoplasmic sperm injection (ICSI) is an effective option for achieving conception. If enough viable sperm cells are obtained through TESE, they can be cryopreserved for use in future ICSI procedures.

TESE Step by Step Process

Step 1

TESE is a minor surgical procedure that is typically performed under local or general anaesthesia.

Step 2

The surgeon will create a small incision in the patient’s scrotum.

Step 3

The tunica is then exposed and a small amount of testicular tissue is excised and expressed. The incision is closed using sutures and a bandage is wrapped around the scrotum.

Step 4

The extracted tissue is then handed over to the IVF laboratory, where it is carefully screened for the presence of viable sperm. This process is essential to ensure that only the healthiest sperm are used during the fertilization process.

Frequently Asked Questions

What is the duration of the TESE procedure?

Unlike traditional open testicular biopsy, TESE is a less time-consuming and cost-effective procedure for patients. Typically, the TESE procedure lasts between 20 to 30 minutes, making it a quick and convenient option for those in need. Recovery time is also shorter, and the procedure is significantly less painful.

Is the TESE procedure painful?

TESE is not a painful procedure, but it can cause some discomfort for a few days. It is performed under local anaesthesia or sedation, so patients remain comfortable throughout the procedure. There may be some swelling and discomfort around the injection site, which can feel like a pinprick, but it is temporary.

What is the success rate of TESE?

Studies have shown that the success rate of micro-TESE ranges from 30% to 65%, depending on the patient’s specific medical condition. The chance of successful fertilization, biochemical pregnancy, clinical pregnancy, and live birth can also vary but is around 85.4%, 22.3%, 22.3%, and 10.7% respectively.

How long is the recovery time after a TESE procedure?

After the procedure, patients can return to normal activities within 24 hours or whenever they feel comfortable. Strenuous activities such as weight lifting and jogging should be avoided for at least two weeks. Patients should return to the doctor’s office for a post-operative check-up approximately two weeks after the surgery.

Can I drive after TESE?

After undergoing aesthetic, judgment can become impaired. As such, it is not recommended that you drive any motor vehicle, ride a bicycle, or drink alcohol for a certain amount of time after the procedure. While general anaesthesia may cause sore throat, jaw discomfort, or muscle aches, these symptoms are typically temporary.
